You’ll need identifying information for yourself and each household member that will be included in your application.

- Phone number and email
You must verify income for everyone in your household who works or receives income.

Tip: Review your MCO before you start
When you’re approved for Medicaid, you’ll choose a Managed Care Organization (MCO), the health plan that manages your care.
Your MCO determines:
- Which doctors and specialists you can see
- Which hospitals are in-network
- Which pharmacies you can use
- What extra benefits may be available (such as adult vision benefits)
